Our prevention services include pre-diabetes and diabetes counseling, weight-loss counseling, and personalized goal setting for healthy living. Our diabetes care team will provide you with the support and education you need to improve your health and lower your risk for diabetes and other serious health problems.

Part of preventing further complications with pre-diabetes or diabetes is keeping up with your care, including taking your medications, checking your blood sugar, and attending regular doctor appointments. This helps you and your doctors get an updated and accurate picture of your health. When visiting Vanguard Medical Group for diabetes care, we have a routine process that we follow to examine all the important factors. This includes:

  • Diabetic eye exam
  • Diabetic foot exam
  • Hemoglobin A1C lab testing
  • Microalbumin lab testing

Diabetes Self-Management Education Program

This month-long program is fully accredited by the Association of Diabetes Care and Education Specialists (ADCES) and provides patients with the tools and knowledge necessary to manage their diabetes and live a healthy lifestyle. Our certified diabetes care and education specialist (CDCES) and registered dietitian nutritionist (RDN) will work together with you to develop a customized plan to improve your blood sugar, A1C levels, and overall lifestyle. This program includes individualized counseling with personalized recommendations from our RDN, blood glucose monitoring instructions, group sessions, and more.
