In the event that you are hospitalized, your Vanguard primary care provider will coordinate your care with the doctors at the hospital. We will remain a core part of your health care team whether you are in the hospital or in a rehabilitation facility or recovering at home.

Hospital care today has become more complex and requires good communication and coordination between the hospital staff and your primary care provider. Vanguard works with the hospital to make sure your health information is available to support your care in the hospital. We track your stay, so we can be aware when you are ready to be discharged home or to another facility for any additional needed therapy.

We get to know the whole person, including family history, lifestyle, fitness, mental or behavioral health and other concerns. We will remain a core part of your health care team whether you are in the hospital or in a rehabilitation facility or recovering at home. We’ll provide valuable assistance for patients admitted to the emergency department, critical care unit, or hospital. We also provide timely follow-up care, including house calls when necessary, after an emergency room visit or hospital discharge.
