Janet Duni, RN, BSN, CCM, MPA

Director of Care Coordination

Ms. Duni is a registered nurse and board-certified case manager with an extensive healthcare background. She has worked in emergency department and intensive care units in major medical centers in Newark, NJ and Philadelphia, PA, and more recently, was a hospital case manager at Saint Barnabas Medical Center in Livingston, NJ. Ms. Duni has non-profit management experience, and for 5 years, was an adjunct professor at Montclair State University in the Health Sciences Department. She joined VMG in 2011 as a population care coordinator associated with a pilot project to support implementation of the Patient Centered Medical Home model, and then became Director of Care Coordination in 2013.

Ms. Duni supervises the VMG Care Coordination team, comprised of nurses, medical assistants and support staff. She creates new strategies and tools to standardize care coordination activities across all sites, and oversees the development and implementation of high risk patient interventions, transition of care protocols, and population management outreach.

Ms. Duni holds a Bachelor of Science in Nursing from Rutgers University, and a Masters in Public Administration and Health Policy, from the School of International and Public Affairs at Columbia University.
