- The New Jersey Academy of Family Physicians (NJAFP) awarded a Patient-Centered Innovation Award to Vanguard Medical Group, a local primary care practice, for its innovative approach to patient-centered care in treating homebound seniors
- Our Verona office was selected as the CPCi (Comprehensive Primary Care Initiative) New Jersey Champion and met with Andy Slavitt, the Acting Administrator for CMS
- 2014 UnitedHealthcare PATH Excellence in Patient Service Award

Our Accomplishments to Date
- Achieved 90th percentile nationwide in 9 of 10 quality measures in Horizon Patient-Centered Medical Home Program
- Participating in the Medicare Comprehensive Primary Care Initiative (Verona)
- Participating with Atlantic Health System’s Medicare ACO (Accountable Care Organization) (Cranford)
- Established on-site integrated mental health services (all sites)
- Implemented care coordination processes to manage high-risk patients and reduce the need for hospitalization
- Reduced by 25% the number of patients readmitted to the hospital within 30 days (Verona)
- Negotiated long-term contracts including shared savings with key insurers
- Integrated a population management disease registry program across all sites
Community Involvement
“Healthcare is local” – Vanguard practices are active participants in the communities they serve.
Each Vanguard office is a member of its local community. Vanguard providers and staff not only provide healthcare to local residents, but serve various community organizations including the school systems, town employees, chamber of commerce, YMCA and Boy Scouts. Our staff volunteer their energy in food drives, visits to the homebound and participation in local fundraisers.
