Laura Tobin Takes Primary Care to Her Patients
“In many instances, my patients become like family to me.”
Laura Tobin worked in home and hospice care for over 20 years before joining Vanguard Medical Group as an adult gerontology nurse practitioner in 2011. At Vanguard, she is the home care nurse practitioner, bringing primary care to homebound patients who can no longer get to the office for primary care and sick/urgent visits.
Vanguard’s home care services include comprehensive physical and behavioral assessments, sick visits, routine follow-up care, immunizations, temporary home care visits due to a transition from a hospital or rehabilitation center, coordination of medical services with other providers, and patient and/or caregiver support in finding community resources, including collaboration with several local palliative care and hospice organizations. Laura maintains continuity of care through follow-up with patients in assisted living, rehabilitation and nursing home facilities.
“Our visits include medication management, regular assessments, sick calls and safety evaluations,” she says. “We strive to ensure that our patients are safe in their home environments and their personal needs are met. We often refer to other community agencies for physical therapy, skilled nursing and home health aides.”
“Home care is a challenging but very rewarding specialty for me because I work with patients in their familiar settings,” she says. “In many instances, my patients become like family to me.”
During her spare time, Laura likes to bike, walk, read and attend classes at the YMCA. Most of all, she enjoys visiting her twin grandsons.
